For your initial appointment, please bring your dental insurance card and arrive a little early to complete any necessary paperwork. If you’re new to the area, we recommend having your previous dentist send us a copy of your dental records.

 

Be sure to bring a list of any medications you take. If antibiotics are needed prior to dental procedures, take them as prescribed. If you’re unsure whether you need antibiotics, check with your doctor before your visit.
